This Family Referrals and Gala Guest Details Policy (this “Policy“) describes how Sweet Journey 17 Foundation, Inc. (the “Foundation,” “we,” “us,” or “our“) handles referrals of families in need and how information about guests at the Foundation’s gala and related events is made available. This Policy supplements, and should be read together with, the Foundation’s Terms of Service Agreement and Privacy Policy.

1. FAMILY REFERRALS

The Foundation’s mission is to house and support the families of hospitalized children and to assist families in need while their children are receiving medical care. This section describes how families may be referred to the Foundation for assistance and how the Foundation handles referral information.

2. WHO MAY REFER A FAMILY

Families may be referred to the Foundation for assistance. Referrals may be submitted by hospitals, social workers, medical providers, community organizations, family members, or other individuals with knowledge of a family’s need, or a family may refer itself. The Foundation may request additional information to evaluate a referral and to confirm eligibility for assistance.

3. INFORMATION COLLECTED IN A REFERRAL

A referral may include the name and contact information of the family and the referring party, information about the hospitalized child’s care and the family’s circumstances and needs, and other information reasonably necessary to evaluate and provide assistance. The Foundation collects, uses, and protects this information in accordance with its Privacy Policy.

Where a referral involves information relating to an individual under the age of 18, the Foundation collects and processes that information only with the verifiable consent of the individual’s parent or legal guardian, consistent with the Foundation’s Privacy Policy. The parent or legal guardian remains responsible for information provided on the minor’s behalf and may review, correct, or delete that information, or withdraw consent, by contacting the Foundation.

4. USE AND CONFIDENTIALITY OF REFERRAL INFORMATION

The Foundation uses referral information solely to evaluate eligibility, provide assistance, administer its programs, and comply with its legal obligations. The Foundation treats referral information as confidential and does not sell, rent, or trade it, and discloses it only as described in the Privacy Policy — for example, to service providers who help administer the Foundation’s programs, or as required by law. Submitting a referral does not guarantee that assistance will be provided; assistance is subject to eligibility, the availability of resources, and the Foundation’s discretion.

5. GALA GUEST DETAILS

Details regarding guests attending the Foundation’s gala and related events — including guest lists, seating, and related arrangements — are maintained by the Foundation and are available upon request. To request gala guest details, please contact the Foundation using the information below. The Foundation may limit or decline to share guest details to protect the privacy of guests, to comply with applicable law, or in its discretion, and any guest details provided are to be used only for purposes related to the applicable event.
